Country Health Connect offers a wide range of community, in-home and residential aged care, early childhood and disability support services. Our services will help you or your loved one stay well, active and independent. Our professional and well trained staff will work with you to find solutions to keep you well and independent at home.

Help at home

Our Home Support Program and Home Care Package program provide all the support you need to stay well at home – simply talk to us and we will do the rest.

Social activities

With Country Health Connect, keep up existing friendships, make new friends, learn new skills and be more physically active with a range of activity groups and home support.

Residential care

We have a number of residential aged care facilities in unique country settings, offering a home away from home.

Disability support

Support and therapy for any age – either in the home or through supply of equipment aids. We are a registered National Disability Insurance Scheme provider.

Respite care

We offer respite care to help you through a difficult time or provide a well-earned break for carers. Our team will help you to work through your options.

Accessing our services

Anyone of any age or ability may be eligible to access our services. If you are likely to be eligible, our team will advise you of the next steps which may include a referral to the My Aged Care contact centre or to the National Disability Insurance Scheme.

How to access our services – Four easy steps

Country Health Connect offers a wide range of community, in-home and residential aged care services to support you or your loved one to stay well and independent in your community. If you need help with everyday tasks or want to discuss other aged care service options Country Health can support you.

From July 2015 the Australian Government advised that to get access to most aged care services you must first register with the My Aged Care service. We can help you with that registration process.

It is fine to have a family member or friend with you through this process if you wish.
