Over 65 years old?
If you are 65 years of age or over, the Australian Government advises that to get access to most aged care services you must first register with the My Aged Care contact centre and Country Health Connect can help you with that process.
If you have already been assessed for services or have been deemed eligible for a Home Care Package, please contact your local Country Health Connect Team to discuss the next steps.
